Giovanni Furnò, 68 years of age, lives in Bollate, a town near Milano. For many years he suffered from cluster headaches, extremely severe and debilitating headaches that recur over a period of time. He tried various forms of treatment, including heavy doses of cortisone injections over many years. Unfortunately, he only later discovered that cortisone treatment is the likely reason why he has developed osteoporosis.

Long-term steroid treatment can seriously affect bone health, causing osteoporosis and increased risk of fractures. The result for Giovanni was devastating. One day in 2001 or 2002, he coughed lightly while eating a meal at the kitchen table. Immediately, he felt severe back pain. Giovanni sought medical attention, and, as shown by various tests and X-rays, it was discovered that two vertebrae had collapsed. Following the operation to reconstruct the vertebae at a Milan hospital, Giovanni had to wear a metal corset for 45 days.

"It was quite a shock when, during this period of recuperation, three more vertebral fractures occured. This second incident was really very severe. I couldn’t even get out of a chair. When I returned to the hospital the doctors seemed surprised. As a result, more tests were done, including bone marrow testing. This last test showed that everything was “fine”, so I underwent further surgery to reconstruct the three vertebrae. In terms of recuperation after surgery, I was not advised to do anything in particular or to take any kind of treatment", he explained.

Vertebral fractures can be extremely debilitating, and for many people of working age fragility fractures impact on the inability to work. In this respect Giovanni was fortunate that his vertebral fractures had not occured earlier.

My work was physically demanding as I was responsible for a warehouse where we had to lift heavy weights. Fortunately, just as my health problems began to cause problems at work, I reached retirement age and was able to leave work with a pension.

He was also fortunate to find an institute in Milano which specialized in osteoporosis. In 2002 he had his first appointment with Dr. Maria Luisa Bianchi, a well known osteoporosis expert. Giovanni was consequently put on osteoporosis treatment and was continuosly monitored through various tests, including DXA scans and periodic blood and urine testing. 

Within six months of treatment Giovanni noted great improvements that have continued to this day. In fact his bone health improved to the point that he could do much of the same kind of activities as he did before the fractures, but obviously taking precautions and being especially careful when lifting anything.

Now, having had medical treatment for 13 years, he has only one intravenous treatment per year and he hasn’t experienced any particular difficulties or set backs while doing physical activity. The only thing he has noticed is that when he sits for several hours, he takes a bit of time to stabilize himself. He also takes great care when trying to stand from a sitting position saying, once burned, twice shy: my second set of fractures occurred because I rose from a chair without supporting myself with my hands as I stood up – now I’m extra careful.

Today I live well, the same as I did before discovering I have osteoporosis. I don’t follow a particular diet, although I was advised to now drink and eat more dairy foods, like milk and cheese, because I rarely consumed dairy foods in the past.

He warns young people who find themselves in a similar situation: Take care of yourself, get better, and be aware of all the medicines you are taking because they can have terrible collateral effects.

Giovanni Furnò, 68 anni, vive a Bollate in provinca di Milano. Per molti anni, precisamente dal 1985, ha sofferto di cefalea a grappolo, che è una forma rara e acutissima di cefalea e i cui attacchi si verificano regolarmente e con carattere periodico. Nel corso degli anni si è sottoposto a numerosi trattamenti, incluse le iniezioni di cortisone.

“Purtroppo, ho scoperto tardi che il cortisone sarebbe stato ciò che, molto probabilmente, mi ha causato la comparsa dell’osteoporosi” afferma Giovanni.

I trattamenti prolungati con questo genere di steroidi possono seriamente danneggiare la salute delle ossa, causando lo svilupparsi dell’osteoporosi e aumentando il rischio di fratture. Il risultato di questa cura per Giovanni è stato devastante. Tra il 2001 ed il 2002, era seduto a tavola quando, in seguito a un leggero colpo di tosse, ha accusato un dolore lancinante alla schiena. Giovanni si è dunque subito rivolto ad un medico per capire cosa avesse causato quella terribile sofferenza, e dopo vari esami e radiografie si è scoperto il cedimento di due vertebre. Il signor Furnò è stato dunque operato all’Istituto Ortopedico Galeazzi di Milano affinché gli venissero ricostruite entrambe le vertebre, ed in seguito a questa operazione ha dovuto portare un corsetto di metallo per 45 giorni.

Non c’è dunque da stupirsi se, durante il periodo di guarigione, il cedimento di ulteriori tre vertebre ha colto di sorpresa sia Giovanni sia dottori che l’avevano operato.

Giovanni ricorda quei momenti: “Questo secondo cedimento è stato davvero molto pesante, non riuscivo neanche ad alzarmi dalla sedia. Tornato in ospedale anche i dottori si sono stupiti di quello che mi era successo. Prima di sottopormi al secondo intervento hanno quindi voluto accertarsi che queste fratture non fossero il sintomo di qualcosa di piu grave, e hanno proceduto ad un prelievo del midollo. I risultati delle analisi risultavano “puliti” ed è cosi che procedettero al secondo intervento per ricostruirmi le tre vertebre. Per quanto concerne il recupero post operazione non mi sono stati consigliati esercizi di fisioterapia particolari e nemmeno di assumere specifici medicinamenti per facilitare la guarigione.

Le fratture delle vertebre possono essere debilitanti, e per molte persone in età lavorativa un elevato rischio di fratture si ripercuote sia sulla possibilità sia sulla capacità di lavorare. Proprio inerente questo tema, Giovanni si considera fortunato poiché le sue fratture non sono avvenute in giovane età.

Il mio lavoro era prettamente fisico, in quanto ero responsabile magazziniere dove gli operai devono sollevare oggetti e merci veramente pesanti. Fortunamente, quando i miei problemi fisici sono sopraggiunti, ero già vicino all’età pensionistica e ho potuto andare in pensione anticipata.

Giovanni è stato anche fortunato nel trovare un istituto specializzato nella cura e nel trattamento dell’osteoporosi a Milano. Nel 2002 ha avuto il suo primo appuntamento con la dottoressa Maria Luisa Bianchi, riconosciuta internazionalmente come un’esperta nel campo dell’osteoporosi. Giovanni è stato poi sottoposto ad un trattamento specifico per curare l’osteoporosi ed è stato continuamente monitorato attraverso ad appositi esami tra cui MOC, esami del sangue e dell’urina.

Gia dopo sei mesi dall’inizio del trattamento si sono visti i primi miglioramenti, che continuano ad oggi. Infatti, la salute delle ossa del signor Furnò è migliorata a tal punto che ora riesce a fare quasi tutte le attività che svolgeva prima di soffrire delle fratture vetebrali, ovviamente adottando le necessarie precauzioni e facendo attenzione ai movimenti soprattutto quando si trova ad over fare dei sollevamenti.

Oggigiorno, dopo 13 anni di trattmenti, Giovanni è sottoposto ad una sola flebo all’anno e non ha più sofferto di particolari problemi fisici durante lo svolgimento di qualsiasi attività fisica. L’unica cosa che nota è che, quando si alza dopo essere stato seduto per diverse ore, ha bisogno di qualche attimo per stabilizzarsi sulle gambe. “Chi si scotta con l’acqua calda ha paura dell’acqua calda, e così anche io: il secondo cedimento delle vetrebre è infatti sopraggiunto alzandomi senza usare le mani come sostegno ed appoggio, ed è per questo che adesso faccio molta più attenzione ogni volta che da seduto voglio alzarmi in piedi” afferma.

Oggi come oggi vivo bene, vivo come prima che mi venisse diagnosticata l’osteoporosi. Non seguo una dieta particolare, anche se mi hanno consigliato di consumare più latticini, come latte e formaggio.

Giovanni consiglia ai giovani di prendersi cura della loro salute e di prestare particolare attenzione a tutti i medicinali che assumono perché possono avere conseguenze ed effetti collaterali dannosi per la salute.
